An good summary of the root causes of 2 horrible, preventable incidents where 17 sailors were killed by what seems to have been really, really bad management, leadership and lack of knowing your job. It almost sounds like they were an amateur crowd pretending to be sailors. 20 years of Submarines, and we knew an awful lof of how our boat worked, from the engine room to the sonar/firecontrol/weapons. These guys almost sounded like they barely knew how to try to point the boat in one direction.
