A few months ago I came across a figure, a bit over $100,000, being an estimate of the most a hospital could claim for one covid case - that is on top of normal billings. Yes, the highest figure required the death of the patient.
I have not seen a breakdown of the components. The headings are listed for us by Joseph Mercola in that EpochTimes story.
and. Which is the actual Federal Department that defines and makes the payments?
The method may be unique to the US. In the UK for example the hospitals (well 98% of them) are owned by government so instructions flow down the chain, they ensure the instructions are kept well under cover.
A curious thought, pervasive as the corruption is (and the cruelty), even the data that does get out gives a reliable picture of what is happening.
